Clean Claim means a properly completed billing form with complete CPT-4, ICD-9 coding or HCPCS coding where applicable, and which contains all the information specifically required in the provider manual. A sample copy of a Clean Claim together with a narrative description of the elements required to deem a claim "Clean" is attached in Exhibit XXX, attached hereto and incorporated herein by this reference.

Make sure to check if there are state required words that must be used. As a result of several class action lawsuits against managed care plans, some states have dictated the specific language that is to be used. Use it verbatim at a minimum.
