Option 1 Supplies %
Supplies. As used herein, “Supplies” shall mean any prosthetics, pacemakers, chemotherapeutic agents, implants, Tissue Plasminogen Activators, other biologicals (e.g., Factor K), and any medical device, implant, supply, radiotherapeutic, and nuclear supply or dye approved for use by the Food and Drug Administration provided or to be used for the benefit of Members. Plan shall pay Provider for Supplies at its usual and customary charge less a  discount of [insert #]  percent.

Option 2 Supplies-Medicare
Supplies. Except as otherwise specifically stated in this Agreement, Plan shall pay Provider separately for the cost of supplies used for or on behalf of Members on the same basis that the Federal Medicare Program pays for such supplies in Provider’s locality as of the date the supply was used.

Option 3 Supplies
Supplies. As used herein, “Supplies” shall mean any prosthetics, pacemakers, chemotherapeutic agents, implants, Tissue Plasminogen Activators, other biologicals (e.g., Factor K), and any medical device, implant, supply, radiotherapeutic, and nuclear supply or dye approved for use by the Food and Drug Administration provided or to be used for the benefit of Members. Plan shall pay Provider for Supplies at the amount charged. Such charge shall not  exceed [X%] over actual cost plus handling fees.
