Identification Card
Identification Cards  Health Plan shall provide Covered Individuals with identification cards that clearly identifies the network.  If an identification card does not clearly identify the network, Payer shall not be entitled to the discount set forth in Product Appendix and will reimburse Hospital at 100% of Hospital’s billed charges, less Copayments, Coinsurance, and/or Deductibles.  Identification cards will also provide the billing address, preauthorization phone number, and customer service phone number.
