“Plan” shall mean any benefit plan or benefit arrangement covering treatment of Covered Services and sponsored by a Payer for whom Health Plan has agreed to provide services. Plan shall include managed care or administrative services provided by Plan for an insured or self-insured Benefit Plan but shall exclude workers’ compensation, automobile liability, Medicare Supplement plans, or plans secondary to Medicare coverage and ALL Healthcare Discount Cards.
