a.    Covered Services Specified. “Covered Services” shall not include the performance or arrangement of any services no specifically included in Paragraph/Section/Clause (insert # of Paragraph/ Section/Clause or section defining Covered Services that were originally negotiated) herein.
b.    Notification by the Plan. Plan will notify Provider of any proposed change in Covered Services that may affect this Agreement or adversely affect Provider’s costs hereunder.
c.    Reimbursement Negotiations. Plan and Provider shall negotiate in good faith an increase in Provider’s reimbursement hereunder and. Where necessary, a new payment method, formula or rate may be required for each additional Covered Service.
d.    Prior Written Agreement Required.  No new service shall be added to Covered Services or to Provider’s obligations under this Contract, and Provider shall not be responsible therefore, until the parties have agreed in writing upon the addition of the service and the reimbursement therefore.
